Sliding Fee

The Sliding Fee Scale is a unique program for our patients at United Health Centers.

The program offers our patients a broad range of services, including medical, dental, pharmacy, laboratory and x-ray.

Who Qualifies:
Those patients who are not eligible for any insurance coverage, and have limited income. Patients pay a fixed fee based on income and family size. A patient’s immigration status is not a concern to United Health Centers.

Based on your income, you may qualify for one of the following categories:
Category A:$30
Category B:$40
Category C:$50
Category D:$60

Example: “Category A” patient has a $30.00 fee covering the visit and laboratory or X-Ray services.

Pharmacy charges will be discounted 70%. You pay only 30% Of charges (This applies to all categories).

Requirements:
The following documentation is required when applying for the Sliding Fee Program:

Verification of Income: (Only one (1) of the following)

  • One (1) month worth of paycheck stubs
  • W-2 Forms from the most current year
  • A letter, on company letterhead, from your employer indicating your gross earnings for the month

    Medi-Cal Denial Letter:

  • Medi-Cal eligibility screening must occur prior to being considered for the Sliding Fee Program.

    Identification: (Only one (1) of the following)

  • Drivers License
  • Identification Card
  • Voter Registration Card

    Verification of Address:

  • Utility Bill

    As a patient of United Health Centers, you will be responsible to:

  • Present all required information for the Sliding Fee Program at the time of service.
  • Re-certify every six (6) months providing updated information. Provide a payment at the time of service.
