Making Appointments
For your convenience, we recommend appointments be made prior to each visit to our health centers. Appointments can be made in person or by phone. Health center hours vary by location. Check here for a location closest to you:

Same-Day Appointments (Walk-Ins)
We also accepts a limited number of same-day appointments for our established patients. Please keep in mind that your wait time may be longer, since patients with scheduled appointments will be seen first.

What to Bring With You
When visiting the health center for the first time, you will need to bring proper identification, which may include a social security card, valid driver’s license, birth certificate or another form of official identification. If available, you should also bring your Medi-Cal card, Medicare card or private insurance card. If uninsured, you will also need to bring a pay stub, an unemployment check stub, a recent tax return or a letter from your employer in order to prove your family income.

Insurance
We accept
Medi-Cal, Medicare, and most private insurances. Our friendly health center staff can get you in touch with Medi-Cal outreach workers and representatives from Healthy Families who can assist patients with enrollment in these programs.

Uninsured
For the uninsured, we accept payment using a
sliding fee scale, based on family size and family income. Please ask our friendly health center staff about eligibility. You will be required to bring your phone or utility bills for proof of California residency.

Co-payment Policy
We appreciate your co-payment at the time of your visit. For your convenience, we accept cash, checks, Debit, VISA and Mastercard. We will give you a receipt for your payment records.

Your First Appointment
To prepare for your first visit, your physician must review your previous medical records. Please arrange to have these forwarded to our office from your current health care provider, and mail or bring to us any records you have at home before your first visit.

Because we take the confidential nature of your health information very seriously, we will give you our Notice of Privacy Practices. It describes how we may use your health information and outlines your privacy rights under HIPAA, a federal regulation governing patient privacy. We will ask you to sign an acknowledgment that you have received this notice. Only individuals given permission on the form may access your health information such as test results or copies of records.

We ask that you arrive at least 20 minutes early for your first visit to complete patient registration. Please bring your insurance card with you.
