Making Appointments

For your convenience, we recommend appointments be made prior to each visit to our health centers. Appointments can be made in person or by phone. Health center hours vary by location. Check here for a location closest to you:
 

Same-Day Appointments

We also accept same day/next day appointments for patients. Please call in advance for an appointment time that can be convenient for you.
What to Bring With You
When visiting the health center for the first time, you will need to bring proper identification, which may include a social security card, valid driver’s license, birth certificate or another form of official identification. Also, you should also bring your Medi-Cal card, Medicare card or private insurance card. If uninsured, it is recommended that you bring:
  • Proof of income (1 months check stubs)
  • Identification
  • Verification of address
  •  This documentation will allow you to possibly apply for the Sliding Scale Program to receive health care services at a discounted rate

Insurance

We accept Medi-Cal, Medicare, and most private insurances. In addition, UHC is a Veteran’s Choice Program Provider and we are proud to service the Veteran’s in our communities. Our friendly health center staff can get you in touch with Enrollment Specialist who can assist patients with enrollment into Covered California Health Plans and Medi-Cal.
 

Uninsured

For the uninsured, we accept payment using a sliding fee scale, based on family size and family income. Please ask our friendly health center staff about eligibility. Please see Sliding Fee Scale Application Requirements.
 

Co-payment Policy

We appreciate your co-payment at the time of your visit. For your convenience, we accept cash, checks, Debit, VISA and Mastercard. We will give you a receipt for your payment records.
 

Your First Appointment

To prepare for your first visit, your physician must review your previous medical records. Please arrange to have these forwarded to our office from your current health care provider, and mail or bring to us any records you have at home before your first visit. In addition, please bring any medications you are taking.
Because we take the confidential nature of your health information very seriously, we will give you our Notice of Privacy Practices. It describes how we may use your health information and outlines your privacy rights under HIPAA, a federal regulation governing patient privacy. We will ask you to sign an acknowledgment that you have received this notice. Only individuals given permission on the form may access your health information such as test results or copies of records.
We ask that you arrive at least 20 minutes early for your first visit to complete patient registration.
